It is a mandatory requirement of the CAA that a medical questionnaire has to be completed online prior to a medical examination being performed by an AME. A pilot must register for a CAA portal account (this can take a few days), and through that portal account apply for a medical. That application is the online questionnaire and health declaration. The CAA levies a charge, payable online, for completing an application.
